Newport retained the two counties silverware on a dramatic night at Aylesbury.
Flackwell Heath missed a couple of good first half openings.
The Swans, making their fourth successive final appearance, took a 67th minute lead through Darren Lynch who converted a Darren Dykes cenre from six yards out.
It seemed enough but with just two minutes to play Danny Jordan levelled for Flackwell Heath after Newport keeper Paul Warnecki spilled a corner.
Extra time produced no further goals, so it came down to penalties.
Jordan and Adam Durrant scored the first two Flackwell kicks as did Richard Armstrong and Liam Smyth for the Swans.
When veteran keeper Paul Warnecki saved Daniel Tripp's third kick for Flackwell it was advantage Newport and Daniel Nti duly fired them ahead.
With the pressure on Jamie West failed to hit the target for the Hellenic side and Scott Mulkern slotted home to keep the trophy in residence at Willen Road and give Newport some reward for a fine campaign.
